Go to admin side and edit Order form page. Choose default.php theme instead of order.php in View tab. I guess I helped.

Or look into the /templates/themes/ directory.
And compare the files inside.
You will see the difference in called Menus.
Now you can add or remove the lines with $sMenuX=$oPage...
and create your personal look (amount of menus) of the page.
