Help to add Customer Input Fields to products_show.tpl

I need to give the customer the option of inputting their name and jersey number specific to each product before they add it to the cart. I already have the necessary fields in the file "products_show.tpl", however I cannot get the "orders_basket.tpl" page to display.

Also, this seems to be more relevant to product attributes, but how can I add these options as an attribute for user input?

please help and thanks in advance...i've been racking my brain over this but not getting very far...

cravenc2372 - If you have QC v.1.x and productAttributes installed you can define 2 attributes in UserInput group and after that assign them to products reequiring that kind of option.
