registration form to shopping cart?

OK I have have a form to be filled out to register how do I get the information on this form to get transfered over to the shopping cart? I normally see a add to cart button .. but I am not sure how to link this information ....Can anyone help?

It's more difficult than if you use the system built-in.

I think it is possible, and easy ...

if you have a "log in" script , to insert, edit, delete informations about your clients and if this script can restrict access to pages .... you can put the restrict access code in the the "ordersDelivery" page and insert a code to retrieve the informations from your "client database" to fill the form.

I having no time to do this now but i will create a client insert form form my QC in future.
